The BRAIN Decision-Making Framework

When an intervention is suggested during labor, decisions can feel urgent and overwhelming. The BRAIN framework creates a helpful pause. BRAIN stands for Benefits, Risks, Alternatives, Intuition, and Nothing, and it turns a rushed moment into an informed conversation.

Walking through it is simple. Ask about the Benefits of what is being offered, the Risks, and the Alternatives such as movement, position changes, or waiting. The I is for Intuition, because birth is instinctive as well as physical, and the N is for Nothing, meaning it may be reasonable to wait and keep watching when mother and baby are both doing well.

For a VBAC labor especially, this framework protects your voice. It helps you and your team stay grounded, understand each choice, and make decisions together, so the birth feels less like something happening to you and more like something you are actively part of.
